BMH 2010-2011 Registration Procedure

Registering your child for Banff Minor Hockey begins in June.  Parents will have two options for registering their child/children in BMH this year:

1.       On-line registration for all programs will begin starting Thursday, June 10, 2010.  If you chose to register on-line, you may choose to pay with Mastercard or Visa or by mailing a cheque or Money Order to the Registrar.  Please note that in addition to providing all your information on-line you may have to send a photocopy of your child’s birth certificate to Banff Minor Hockey to complete your registration.  This copy will be kept on record for future years with BMH.  The deadline for on-line registration is:  Wednesday, September 15, 2010.  After this date, registrations will still be accepted up until September 30 but space may be limited.  To register on-line, check out the information under the On-Line Registration Tab which will be available June 10th, 2010.  Currently the forms required for registration must be downloaded and mail or dropped off at Gourlay's Pharmacy until we can create PDF writeable files.

2.       For those of you who prefer not to or are unable to register on-line, we still have registration night at the Banff Scout Hall on June 9th, 2010, from 7:00pm – 9:00pm.  You should bring a photocopy of your child’s birth certificate, to the registration night (this copy will be kept on record for future years with BMH), your child’s Health Care Number, and a cheque, cash, or money order to complete your registration.
 
3.       If neither of these options work for you and you need to send in your child’s registration by mail, please ensure that it includes the following: (1) a completed Registration Form, along with the Town Of Banff User Form, (2) a completed Internet Disclaimer and Consent Form, (3) a completed Volunteer Form, (4) a completed Medical Form and (5) a cheque or money order, and (5) a photocopy of your Child’s Birth Certificate (this copy will be kept on record for future years with BMH).  All mailed applications can be sent to:  Registrar, Banff Minor Hockey Association, P.O. Box 1954, Banff, Alberta, T1L 1B7.  The deadline for receiving applications is September 15, 2010.  After this date, registrations will still be accepted but space may be limited.
 
Payment in full for all programs is due by Friday, October 1, 2010.  Any payments received after this time will be subject to a $50 late fee and will be treated as a late registration.  In this case, the Board has the right to place the player based on program availability.  If payment for any program is not received prior to the commencement of the Hockey your child will not be permitted to play until payment is received in full.
